Hi,
Does anyone have a performance improvement plan template that incoporates performance management and quality improvement that they are willing to share? OR does anyone have any tips on how to update an existing QI plan to turn it into more of a PI plan?

The Minnesota Department of Health has a 1-pager that summarizes their thinking about the relationship between performance management and quality improvement. MDH also has a site that features recorded webinars on those topics. Both resources provide some insight about how an agency could organize their QI and PM activities.
