Does anyone have a Quality Improvement Plan that was created based on PHAB Standard 9.2 requirements? We've folded our QI Plan into our Performance Management System Plan but are thinking of creating a separate document that very specifically addresses the PHAB version 1.5 Standards and Measures. If you've done something along these lines, please share! Thanks so much.

Our QI Plan and Performance
Our QI Plan and Performance Management Plan are closely linked, but are two seperate documents. I took what I liked best from all the QI Plans that I could find posted and also made sure it complied with version 1.5. Happy to share both, just email me at bartowr@stlouis-mo.gov
