Posting Your Agency QI Plan Again

Hi,

In 2012 and again in 2014 PHQIX members uploaded their QI plans to the Community Forum to share with others. 

Most of these QI plans are now outdated. Are you willing to share your current agency QI plan with others on PHQIX?

There are some excellent templates and resources in the Appendix of the Dayton and Montgomery County Plan. I really like the "QI Project Proposal Form" and the "QI Council Project Criteria Worksheet". I'll definitely be using these with QI project teams in the future!
