Hello,
We are looking to change the way we report and share our CHA. Rather than publish static pdfs and link them to a webpage, we are interested in using data visualization software to create a dashboard and other interactive charts and text. If you've done the same with your CHA or are investigating ways to do this, could you please contact me? I'm very interested in learning from others and seeing examples. Thanks so much,

We just released our online
SHA Visualization
We just released our online SHA tool to complement our SHA PDF. It is part of our overall SHA/SHIP website: www.odh.ohio.gov/sha-ship and can be found specifically at: https://healthfacts.ohio.gov/
The team used Tableau to build all of the tables and visualizations.

Although we do not have it live at this time, we have been utilizing the Clear Impact Scorecard (based upon Results Based Accountability) for a variety of things including our SHA, SHIP, Healthy People, etc. Since our health dept does not have a cool queryable data warehouse, we have no way to provide access to dashboards on a larger scale. The Scorecard is a performance management system so has cool functionality to provide visibiliyt of both population measures and program measures; and link them together. Glad to share more about the system and our experience.

The Philadelphia Department
The Philadelphia Department of Public Health shares CHA data through the Community Health Explorer. The Community Health Explorer was created using Tableau and is managed by the Philadelphia Office of Open Data and Digital Transformation. It was launched in 2016 and datasets are updated annually by the Philadelphia Department of Public Health.

We are also in the process of doing the same thing with our CHA, but have not finalized it yet.
We found that many of the performance management softwares on the market can help display a dashboard with CHA data measures (population health statistics) and if you were trying to display progress on your CHIP strategies and goals. This community forum thread has a lot of those software titles (AchieveIT, Klipfolio, InsightVision, VMSG, etc.)
However, if you are looking for something more visually appealing with graphs, etc., you may want to check out livestories.
